OK here is the job offer where I will be working as a new PNP and things discussed in the meeting, any suggestions

Wanted to re-cap our discussion in regards to your accepting employment at the Clinic site in the capacity of FNP in Pediatrics.

Discussion held on treating patients birth to age 23; Dr. J suggested that I speak with Dr. B to seek her cooperation for assistance/consultation for patients over the age of 18. I will be speaking with Dr. B in the next few days. I don't anticipate Dr. B will have a problem with this.

Discussion was held on protocol in the event that a pediatric patient needed to be hospitalized. Dr. J wishes to be consulted when you feel a patient is in need of inpatient care. Patients will be referred by Dr. J to her colleague Dr. C who covers herl. In the event Dr. J is out of town or otherwise unavailable she will notify Dr. C of her absence and in that event he would be expecting the call directly from you.

Discussion was held on newborns. Dr J stated due to Dr. M being an OB/GYN at the site you may be getting newborns. Dr J stated she would rather do the initial visit and then be consulted periodically thereafter. Her reasoning is that with newborns it is often easy to overlook something and two heads are better than one when dealing with them.

Discussion was held on prescribing; you will need to apply for the Illinois Controlled Substance license and a DEA. The Corporation will front the cost of the DEA and deduct it from your annual CME allowance.

Dr. J doesn't want you to write scripts for the ADD/ADHD children but rather contact her to do so.

Discussion was held on you sitting for your test on Wednesday, December 16. You related you would notify me on your way home of the outcome of the exam.

Discussion ensued on your availability to begin employment on Monday, January 11, 2009. You will start seeing patients on Tuesday, January 19th at the office. The office Manager will work with you on a template on how you prefer your patients to be scheduled.

Your orientation to Corporate issues/policies will occur on the 11th beginning at 9:00 a.m. at the Clinic site. The Human Resource Director has verified the date and time. She will prepare an agenda for the meeting as you will be meeting with most of our department heads throughout the day.

The first week's schedule will be as follows:

Monday, 1/11/10 Orientation at Clinic

Tuesday, 1/12/10 Shadow Dr. J

Wednesday, 1/13/10 Go to Office Manager and will meet you for introductions,etc. You might want to read through Dr. K charts, etc. sit up office

Thursday, 11/14/10 Shadow Dr. J

Friday, 11/15/10 Go to site

I will await the State of Illinois Credentialing form, etc. As soon as I get them we'll get busy enrolling you into our system while waiting for your actual license.

Salary is 36.00 hr after 16 patients i get a bonus of 10$ per patient, cme 1250 with five days a year for my cme a year 21 paid day vacation plus 6 sick days, a nice office plus student loan repayment 25,000 a year, have been assured that I will have plenty of go to people mid levels and md's they pay my malpractice and cover my tail, it is very rural medicine and am really excited

In IL, you can't write for class II meds anyway (Ritalin, Adderal, etc). This has passed the IL senate and is up for review by the DEA so we (APNs in IL) fully expect to have these prescriptive priviledges next year.

Also - if you haven't joined, please consider joining the IL Society of Advanced Practice Nurses (wwww.isapn.org) - this is your organization that protects your practice rights in IL.

you are correct, I had forgotten about that and one friend thought that it was strange that I would not be writing class II meds, and thanks for your suggestion about isapn I belong to 3 national associations I am pretty excited about the job but a little nervous at the same time. I do not feel you should be limited on what conditions you treat, such as in the case of ADD/ADHD. Once you are able to prescribe these meds with your license, you may not want to hand these off to the physician. In time, you will become proficient with these patients and not need MD's help.

I am a FNP. I thought your certification as a Pediatric NP limits your license, certification to include up to age 18 only. Your classes were limited to pediatrics.

Benefits sound great!You may want to ask about hours, max number of patients daily.

pnps can see up to age 21 for those in college, and age 23 for those with special needs according to NAPNAP, if I am wrong please correct me, I know of a fellow nurse who is type 1 diabetic and pregnant who is still being seen by her pediatrician, my understanding from her is that he has a great interest in Diabetics, but I strongly feel that he needs to let go of her and let the specialists handle her care at this point, but she sees nothing wrong with this but I think that is just plain weird;

I work in an ER prn and one of the peds in that area sees pts until they get married which I thought was kinda weird.
