What's everyone's thoughts on this?

More classroom training? More clinicals? Or pehaps a residency like physicians complete?

Would be interesting to hear everyone's views. I'll also post a poll for those who wish to vote.

BTW, I'm in favor of a residency. GOSH how one would have helped me my first year out!

Think all NP's first year should be residency, esp if working in a hospital acute care unit.

I figured we would pass the costs on over to medical residency department of the hospital. I know it might bring down the doctors saleries, but com'on... NP's are better anyway.

residents are funded by medicare - the government spends about $130,000 per resident every year - the hospital/department in turn pays the resident $30-40,000 and keeps the rest to cover departmental expenses/medical malpractice/etc....

I wasn't aware of that. Always assumed the hospital took on the resident and their funding.

Ofcourse, they could train NP's for a year, for less money, and get just as good of a product.

NPs cant replace doctors. NPs and MDs need each other. No one group is better than the whole.

hospitals make a crap load of money off of residents. Its a huge cash cow for them. You are naive if you think that the money just gets used up training the resident. Its an understood reality that Medicare funds each resident position in excess to reimburse hospitals for patients who cant afford to pay and have no insurance. Most resident training programs are in hospitals that ahve a large portion of uninsiured patients.

See, it's not so hard. I'm an agreeable person!

I'm am not in any way saying that MD's cannot be replaced. I wouldn't want them to, half of my family would be jobless!

MD's are 100% needed for speciality fields.

Would I let a midwife deliver my child? Maybe. Via a c-section? Absolultely no.

Would I let a NP or PA perform a CABG on me? Nope. I want the most highly trained CT Surgeon on the planet.

Neurosurgery? Nope. Still need a pro.

However...

BP through the roof? NP

ER presentation with CP and R/O MI? NP

Routine GYN? NP

Chronic pain? NP

Acute pain? NP

GERD? NP

OA/RA? NP

Septic infection in ICU? NP

Np's can and DO provide superior care. MD's do as well. DO's do as well.

A monkey can do those things. Long term, neither NPs nor MDs will be doing that basic stuff. You dont even need to go to college for such basic practice.
