We have this system for reporting issues anonmyously and I am so turned off by how it is used that I do not want to work here after I graduate. It seems like it is used to tattle behind people's back or blame someone else for something is done. It's not really accountability for anything.

I've done one and that was a mandatory one. A patient fell. However, it seems like people fill them out every shift. It is ridiculous! Oh, you don't like what someone did? *name of program* them! Oh, this BS happened? *Name of program* them!

Like the other day some how the downtime procedure was messed up because we had two new girls. They couldn't figure out what to do. I was told to *name of program them* and I almost did, but I decided to screw it. It wasn't that bad and they are newer. I think it was the first time they worked the downtime without the usual full-timer. I said screw it. Although they did mess up the next downtime, but the manager knew they needed help and further training based on how many times they called her.

At the end of the day, I get over things fairly quickly. One time the patient care tech dumped an EKG on me when I was busy sorting out a mess of an admission as the clerk. I got over it.

Now we have a new policy regarding something stupid like whenever a patient leaves the department, we have to fill out a ticket to ride involving unnecessary paperwork because the transporters and the RNs were duking it out with the reports.

Honestly, I am sick of this program and how freely people use it. It's like, really? You're doing a report over that? It's silly little petty things. I think there were a bunch that were written about me that I do not know about, but this is ridiculous. I do not really want to work in this hospital system that has this petty little system. If it was used for serious stuff, that would be different. However, the way it is used it seems ridiculous.

Most all *Name of Program* are "usually" designed to be a risk management tool, and is not "supposed" to be retalitory in nature.

It is supposed to assist in policy, re-education, safety issues that need looking at.

What it ends up being is an incident report/specific complaint against supposed not up to par nurses in practice.

It would help to have a re-education of what the *Name of Program* is supposed to be, as opposed to what it is seemingly being used for at present. And what the policy is for those incidents that are specific to complaining about other things ie: a nurses practice, etc.

Well, I am not entirely sure what it is supposed to do compared what it is being used for. Like if equipment fails, you need to report that. If a person falls, you need to report that. One example that was given to me was that there were issues with the patient not being on telemetry because the nurses never hooked them up but blamed the other department.

It may appear to be like "blaming", but jadelpn described it correctly; it's a risk management tool. It's not a disciplinary tool; and maybe people are using it as a CYA tool; maybe a punitive tool.

I agree with jadelpn in a reeducation session; find out from risk management what applies in reporting.

A lot of times people believe "anonymous" reporting is negative; when utilized appropriately, it helps resolve issues to ensures patient safety and a safe workplace.
