   I im not mistaken, with regards to student insurance, its payable to ltu. Its clearly stated in the form anyways when you get your letter offer.

   For my vaccination record, I submitted a certified photocopy of my baby book and for the recent shots that I acquired, my doctor just wrote the generic and brand name of the vaccines and the date that they were administered.

   Does your TOR/RLE include the list of hospitals and training centers where you had your clinical placements? It should also contain the number of hours spent on duty with each specific institution. Hope that helps.:)
