   I just felt extremely bad but I do know that it is my fault for not double checking his IV. Is it normal to make mistakes? It's just that I doubt myself now because of what happened. This is the second time that I made a mistake. I felt irresponsible...

   He did not have an iv access the whole shift. I haven't got the chance to assess him because of the back to back admissions. Now I'm feeling bad about myself as a nurse. I am starting to doubt myself as a nurse.
