I have heard of vascular cellulitis or at least have seen many but OBITAL. how common is this? I guess I should look it up but just curious if someone has had a pt who had this.

I had a friend/patient who i took to the eye doctor one time with a horrible eye infection. He promptly was admitted to the hospital and put on IV antibiotics. It came up overnight and he didnt really know how or what happened. Not sure what cultures showed, just know the eye doc was really concerned about his eyesight and him possibly losing his eye if he didnt get immediate antibiotic treatment. It did go away and as far as i know never came back, but it sure looked horrible.
So , it was orbital,, it was cellulitis,, strep i dont know

A lot of the orbital cellulitis that used to be seen was related to HiB, so it's not quite as common now that vaccination has been implemented.

I work at an Urgent Care on call. Any facial cellulitis, especially around eyes or even mouth, nose, etc., is treated pretty seriously. Very close to that important thing, ummm, let's see, what is that?? oh yeah, the brain.
We may do a culture in the clinic, but of course this takes a day or two to come back, so don't know what the bacteria is that is causing it. We often give a gram of Rocephin IM for facial cellulitis and tell the patient that if it isn't improved at all, that they need to come back or go to the ER, usually for admission to the hospital.

The husband of one of my nursing school friends had cellulitis in his elbow...they cleared that up..or thought they did..and he turned up with cellulitis in his eye orbit. Equaling 2+ weeks of antibiotics in a picc line...home health,the whole shebang. total mess
