ewwwww....ive been a nurse about 19yrs and ive worked in careplans now for about 2years and i still hate to do care plans ..lol

anyone one else work in careplans, if so do ya like it?..lol

We have communication sheets + care plans! I have this policy in that I'll write either the care plan or the com sheet but I will not write both! So either I write "care as planned" in the com sheet (then put entries in the care plan) or I'll write everything in the comm sheet. However, truth be known - I prefer the communication sheet where I write everything that's been important for the shift & will be of interest to the nurse taking over - it's Sooooooooooooooo much easier than turning over sheet after sheet of the stupid care plan! :-)

I know this is an old thread but I was just browsing trying to understand the US system and work out all the abbrevs!!

I just spent about two and a half hours constructing a care plan,not bad you say but this if for someone who will only be in a week!!We used to have shorter versions for respites but nowadays we don't differentiate between respite and permanent as it is all "care". Does anyone actually have the time to read them,add to them etc before the person has gone home again?

It depends on what you're updating but the easiest way to update is with doctor orders. We go to the nurses station every morning and pick up copies of the telephone orders and update the care plans fromthem. Hope that helps a little. ;)

I need brushing up how to write new orders to careplans, can you help?:confused::confused::confused::confused:

Example:

New order: Septra 10 days for UTI

Careplan problem:

Resident has UTI

Careplan Goal:

Resident's UTI will be resolved upon completion of ABT AEB negative ua/cs

Careplan interventions:

1. ABT as ordered

2. Encourage fluids

3. Monitor for adverse effects from ABT

4. UA/CS as ordered upon completion of ABT

I love to write care plans...call me crazy but I love the process and coming up with realistic goals and time frames for the residents. Great hint on the last post but I believe the CDC guidelines do not support post antibiotic testing in an asymptomatic person.

:trout:to me... but the reality of my life is that most MD's write the order to do the follow up u/a therefore I usually use it since I know I'll have it. But, you are right!

Not to go off on a tangent, but you know that we as nurses have to keep educating the docs. Half of them where I work now have never heard of the Beers' List. If you mention to them about the CMS guidelines for wound care they look at you blankly. And a few of them think the CDC has to do with a new form of downloadable itunes. I have nurses who won't ever question what a doctor says!! I have fallen into a time warp and am back in the 50's!

I DO TRY to educate Dr's., but we've got a couple that... well I'm sure some of you have 'em too.
