I'm hoping someone can help me figure out a NANDA diagnosis to use for my care plan. Usually I do not have so much trouble trying to find one but it seems like the only one I could come up with is risk of falls. So here's the situation. I have a 15 yr old male who dx with hallux valgus (bunion) and had surgery (calcaneous osteotomy with resection of the right foot). The risk for fall I thought would be good because he is non weight bearing for 6 weeks with a cast. But it doesnt have any defining characterists to use only related factors. I was also trying to find one that had to do with possible risk for injury but that one only has related factors as well. If you can please try to steer me in the right direction that would be great !! :nailbiting:

Skin inegrity, infection, falls, pain. Those all seem to fit your situation. Do you cluster concept map. I find that helps me when I'm stuck.

The patient has a cast on. Think about what you want to monitor for and have the patient monitor for with a cast on. Think along the lines of tissue perfusion, swelling, pain...

Hope this helps. Google will help out too.

A few questions:

What is your systems assessment state for your pt?

Do you have a care plan book?

As the greats on here state:

"Tell me more about the pt."

Nurse wise; take the medical diagnosis out of the pt.

All risk for diagnoses only have R/T factors not a single one has defining characteristics as they are risk not actual. If they meet the R/T characteristics the NDx applies Only actual diagnoses like impaired gas exchange have defining characteristics.

With prolonged immobility comes... High-risk of a certain emergency...

See thats the problem I'm running into because he was basically just at risk for everything. And their are no defining characteristics for that. His pain was low besides that ones tricky to do anyway. He had no abnormal on assessment. Im at a loss.

There are NEVER defining characteristics for risk for diagnoses only risk factors.

I know thats the problem lol
