What is the discharge and addmission process in your rehab hospital?

I am a new grad LVN and I am currently on orientation. I understand the med process and most of the documents. At the moment I feel my weaknesses are in admissions and discharging.

The reason for this is I haven't really done this yet. Anyone have tips or sample documents?

I have worked in home health and LTC and most LPN'S dont do admission but I think its state regulated and I live in PA. I can NOT do an admission any place and discharge i can do as long as the pt does NOT have medicare/medicaid. But in any event you can do it, just take your time and you will be ok

I work at a freestanding rehab hospital in Texas. LVNs have a wide open scope of practice here, but the policy of the facility forbids LVNs from completing admissions. However, when I worked in nursing home rehab (SNF), LVNs regularly completed admissions.

At my workplace, the RN completes the admission assessment and initial plan of care, while the LVN notifies the physician of the patient's arrival, verifies admission orders, writes the med/treatment orders on a physicians' order form, hand-writes the first MAR, and administers the initial doses of medications.

LVNs frequently discharge patients at my facility. However, since I work the night shift, I am not really involved in the discharge process since we rarely D/C patients to home during the nighttime hours.
