I have just looked at the CGFNS website for the Visa Screen Certificate, and there is only mention of the CGFNS exam or the NCLEX-RN exam to meet qualifications. If a nurse has passed the CGFNS exam and the English exams, then they meet the reuqirements for a green card based on that. At times, there have been nurses that have been unable to pass the NCLEX-RN exam and have taken the NCLEX-PN exam and can work as an LPN. But they have already met the requirements for the green card based on the CGFNS exam.
Immigration will not issue you a working visa with the LPN Diploma. Even Canadian nurses cannot get work permits for the US as an LPN.........RNs are covered under the NAFTA TN visa from Canada, but LPNs are not.

Complete the enclosed forms to send to your nursing school(s). Academic transcripts must come directly from the nursing school(s).
Complete the enclosed forms to send to your licensing authority(ies). Validations of all licenses, past and present, must come directly from the licensing authorities.
You must successfully complete an English language proficiency test and have the scores directly forwarded to ICHP from the testing agency.
You must successfully pass either the CGFNS Qualifying Exam or the NCLEX-RN® examination if you are a nurse.
Enclose a copy of your high school diploma or equivalent.
