Does anyone know the best book to create Care Plans.. Please help

As a wonderful note for future reference--in the hundreds of care plans that I needed to create in LPN school, it was not in my scope as a working LPN. I just followed the care plan. It is, however, helpful to create them in school, as then you get the idea on where the nursing treatment is going for your patients.
