Hi.I downloaded the visa screen handbook in their website, http://www.cgfns.org/files/pdf/apps/VS%20Handbook%20Web.pdf

Im having problem filling out my visa screen application form. i really have alot of questions!

1. In Preliminary information (1). about item letter D, i dont have any work experience yet. because i just graduated and passed NCLEX. what will i write here?

2. Other names (3). since i dont have any other name, should i leave them blank or just put N/A or none?

3. Occupational Visa Information (12). from h-1b, h-1c, 212®, TN(status), Permanent (green card). which one should i choose? i dont know any of them except for the permanent.

4. Educational Evaluation (14). Professional Education (b). i attended 3 schools. 2 of them are non-nursing schools. i didnt finished my course on the 2 non-nursing schools. i just transferred. tell me if this is the right thing to do - ill list all 3 schools in the form. then submit only the integrated TOR from my last school of which i graduated from.

5. English Language Proficiency (17). I havent taken up this exam yet. it says in the form that i can submit the vsc form prior to english exam. so i just have to leave them all blank? i wish there's a check box that says "havent took the exam yet" .

Green card is another name for the permanent visa, that is what you are applying for.

If the other names does not apply to you, then put N/A.

If you do not have experience then write new grad.

Your transcripts that were submitted for you to apply for licensure is what you need to submit to them. You can list the non-professional programs but the transcripts are not needed from there.

I suggest waiting on the VSC application until you know when you are taking it and that you will pass it then. The application is only good for one year, and with the retrogression in place, it will be several years before you have chance to get to the US; it is no longer guaranteed that you will get the green card. There are many more applicants than there are visas available for those from your country.

The VSC is currently needed when you have your interview at the US Embassy, not needed before. Would focus on passing the English exams before submitting the application for the VSC.

Hi! Just a follow up question, I'm having trouble on answering item number 14 Education Evaluation, particularly the "professional education" table.

I dont know what to write on the "Professional Title Obtained". When i graduated college i obtained a degree bachelors of science in nursing, but its not considered to be a profession right? so what should i write on that column? Nurse?

And in the column "Name of Diploma or certificate in its original language". i don't know what to write there.. "[name of school] Diploma? Certificate of Completion? All i can read in my diploma is "...... has conferred upon [my name] the degree BSN in recognition of the satisfactory fulfillment of the requirements pertaining to this degree and endow him with all the privileges and onors thereunto belonging."

Help please..

BSN is what you want to write there, that is a professional degree that is issued to you.

And for the other line, they want what is written on the line in the main part of the diploma, and that is the BSN again.

Registered Nurse is not the name of the degree, as the degree does not give licensure, but the BSN is what is to be used.

Hi! i have this question, i dont know if im gonna fill up the form "Authoriztion To Release Information" since i dont have an agency and im processing my VSC on my own, do i have to fill this up?

Im confused because someone told me that i should fill this form with the information about the state on which i got a license like for me its in New Mexico, so I was told to fill this up with that because New Mexico will be needing information about my application on visa screen. Is this true?

And btw where should i put my application for mailing? is it in a brown envelope or just a normal small envelope?

No, you do not need that Release Form. That Form is only for any 3rd party, such as agencies to get a copy of something from a BON or from CGFNS.

If you are applying for VSC, New Mexico has it's own License Verification Form strictly for CGFNS purposes. You only need to download it from their website and send it back to them, with the fee.

We recommend using courier (Fedex, DHL, etc..) for any thing that will be containing a bank draft or money order.

If you prefer Philpost, just use what is approved by them.

It really doesn't matter what to use, so just use the one being used by the courier or the local post office since it will only contain paper application and no fingerprint cards. Any package containing a finger print card need to be in hard envelope to prevent folds and bents to the card.

And if you will be paying via credit card, you don't even need to mail it. They will accept it via fax. Just make sure the info. on the credit card is accurate.
