I am trying to obtain an Arizona RN license. I am a canadian nurse who has written NCLEX and worked in NM about 12 years ago. I have tried calling and emailing azbn but getting a valid response is difficult. (I do realize there are probably 1000's of applications they are trying to deal with). I have received conflicting information as to whether or not I require a visa screen from CGFNS (even cgfns were not sure) as I am applying for a TN visa not HN-1. However I need a letter of employment for my TN and I-94 but can't get employment without a license. Any information would be greatly appreciated.

I am in the exact same position. I am a Canadian RN with 4years experience in Ontario. I passed my NCLEX but don´t know how to obtain my license if I don´t have citizenship documentation until I have a job offer. I don´t know what to apply for and no one seem to know at the State Board. Have you found out yet?

visas for mexican and canadian nafta professional workers

if you worked in arizona as rn before, would need to reactivate prior licenese, cgfns only needed for first time license applied; az now accepts canadian licensure board information.

use this form for license reactivation: rn/lpn renewal application

see section:

citizenship/lawful presence documentation required:

federal law 8 u.s.c. 1641 and a state law a.r.s. 1-501, placed into effect 1/1/2008, require documentation of citizenship/nationality/alien status for certification. if thedocumentation does not demonstrate that the applicant is a united states citizen, national, or has alien status, the applicant will not beeligible for certification in arizona. all applicants must submit documentation regarding their citizenship/nationality/alien status once after that 1/1/2008 date unless submitting a document with an expiration date (excluding us passports). if documentation has not yet been submitted, see list a & b for specific documentation required. list a & b can be found at www.azbn.gov/application.aspx by clicking on 'citizenship and or lawful presence cover sheet/alien status declaration/lists a &b' under other form downloads. a

photocopy of the documentation you submit must be on 8 ½ x 11 paper.

arizona will accept as "validation of educational requirements"

have the canadian licensure board submit a passing score on the english

language version of the cnats or crne (canadian licensure exam)

and verification of canadian licensure status directly to azbn.

rn/lpn application by examination has contact information at bottom of form who you can call for help.

For TN visa you will be required to have a visa screen certificate and you just need the state to confirm you passed NCLEX. Many employers have issued a letter of employment knowing that the license will be issued once in the US with a valid work visa and a SSN has been issued

I have applied for my visa screen and reactivation of a previous MI license to assist with getting my license by endorsement in AZ. I have also applied for my visa screen and am now awaiting the different agencies to submit information in my behalf. I have been told by recruiters that once my license is to the point of just requiring my visa/immigration papers and my visa screen is complete I will be offered employment and then as soon as I enter the us and receive my TN + l-94 and submit to AZ the will issue a temporary license - is this correct as far as anyone knows??
