Re-admission orders not done for TWO DAYS!!! (seriously??)

Wow, I am still in shock over this! Resident re-admitted after surgery, re-admission orders NOT DONE for two days!!!! :no: Not only that, the nurse who re-admitted them didn't check the surgical dressing or do a head-to-toe assessment. This resident got their regular daily dose of COUMADIN **twice** before I caught the error----I'm sure when they see the surgeon for follow-up, he'll be SO happy about that his order to HOLD the Coumadin wasn't carried out for two days....NOT!

Annnnd, the surgeon also changed the Metoprolol (he reduced it by half), so the resident also got two days of their former, higher dose. Did anyone take vitals after the ONE time when the resident first came back? Nope!!!! Our director doesn't seem too worried about it, (the nurse who readmiited the resident is known to "ignore

admission paperwork" and it's totally accepted).....can you imagine if the resident's spouse and/or family, or if STATE finds out???!!!

Thank goodness when I assessed the original surgical dressing on day TWO of them being back, there was only a small amount of reddish-brown drainage. Since it's the original dressing and they see the surgeon again in a few days, I just reinforced it with extra non-stick gauze and obviously kept an eye on it. No one else assessed the dressing for another two days....really????:arghh:

I was so upset by that and other things I saw in my last several shifts, I was almost in tears over how bad some of the care is, and it made me even MORE determined to keep my Dad and/or other loved ones out of a Nursing Home no matter what! :no:

First question is: What is the nurse-to-resident ratio in your facility? Very often it's so ridiculously high that the nurses can't provide good nursing care.

This has happened to me before..i work 11-7 nights. The pt was readmitted from the hospital..no new orders had been transcribed,

No new mar written out..the evening nurse expected me to write her entire new mar

and give her evening meds at midnight.

I don't think so.

Your situation is completely unacceptable..i would go further than my DON until i got answers. State would freak out.

This DNS would freak out! We would not allow this in my building. Nervousnurse, not all nursing homes are horrible.
