E-Kit Question Need Advice

I am a Pharmacy Coordinator for LTC. We have techs who put together E-kits for start doses and emergencies. The Nursing staff are supposed to fill out a little form when they use something and send it to the Pharm, but they rarely do. When we send staff out to the facilities they replenish these empty boxes, but it's causing a big problem. Can you give me some advice on what you do at your facility? Any technology updates, or special policies. Any help would be greatly appreciated.

Every facility I've ever worked in has had that same problem. When the pharmacy calls and tells the supervisor or DNS that the FACILITY will have to pay for the doses, somehow everyone remembers who used the 40mgs of Lasix or whatever.
Perhaps a bar code reader on the kit with a code for every patient and a code for every med??? One quick swipe and you would know what was used and for whom.

Our Pharmacy has duplicate sheets we fill out and fax over when we borrow from the E- or C- boxes. Then the Pharmacy knows we need a box replaced. The sheet includes pt. name, room, what was used, etc.

That actually isin't a bad idea with the bar codes. Some places reccomend using a computerized system like the hospitals do. My biggest problem is with compliance. Sometimes those forms just don't make it back. Today I had a bad glucagen problem b/c someone forgot to send the triplicate in.
