Putting all the pieces together & tissue perfusion questions

Hey all! Just started my 3rd semester in the ADN program. Our care plans this semester are expected to be very focused on patho. and I am having trouble pulling all of my pieces together. For example, the kidney's effect on the heart, lungs etc...bla bla bla. I have most of the necessary information but when I go to write my care plans, I have a hard time relating all the information to each other. My instructors believe we have the knowledge to put the pieces together on our own and they get upset when we come and ask for help. Does anyone have any ideas at all?? Any good books, websites, tips??

Also can someone explain altered tissue perfusion to me?? What exactly does altered renal tissue perfusion mean? Altered cardiopulmonary tissue perfusion? When I think of tissue perfusion, I think of circulation and I know that not all of these perfusion problems are related to circulation. My fellow students seem to understand it but they can't explain it to me. Can someone help??

Hey! I think they all are related to circulation. The blood carries the oxygen to these organs so that they can function. If you have altered perfusion then that means that the blood is not able to perfuse through the organ meaning that oxygen can't perfuse through the organ. If they don't have enought oxygen then they can become hypoxic and can die. I hope that makes sense. I don't know which med surg books you have but mine are very helpful in understanding. I have Medical-Surgical Nursing by Lewis, Heitkemper, and Dirksen (6th edition)

I think I'm saying basically the same thing as nurse daisy, but in a different way...

Altered renal perfusion means that perfusion to the kidneys is altered. This will have a profound effect on BP, urine output, RBC production, etc. Look at what the kidneys do and you'll know how poor tissue perfusion can effect other organs. Poor cardiac perfusion would result in myocardial ischemia and risk for myocardial infarction. HTH.
