What are your ideas?

Do you have a NANDA reference book?

The way this works is that you show us what ideas/starts you have for your homework questions/careplans, and a generous nurse or two may help you flesh out your ideas.

The way it doesn't work is you asking and receiving instant care plans.

You nursing diagnoses are based on your patient assessment, not on the medical diagnoses.

What are you patient's deficits and strengths? Does she have poor support? History of depression? Feelings of helplessness or self-harm?

What are her coping skills or stressors?

Yeah, Nursing Dx doesn't work that way. First step is to do an assessment. THEN, cluster assessment findings & map those 'defining characteristics' to the appropriate Nursing Dx.

Please memorize the ANA definition of nursing.... found in the Scope & Standards of Practice. "Nursing is the protection, promotion, and optimization of health and abilities, prevention of illness and injury, alleviation of suffering through the diagnosis and treatment of human response, and advocacy in the care of individuals, families, communities, and populations."

As you can see - our practice focus is on human RESPONSES rather than the illness or disease itself (that's the scope of Medicine) and this is the basis of Nursing Dx. It may seem kind of squirrel-y now, but as time goes on, it will make more sense.
