I have to make a concept map, but my instructor didn't tell us how to do it. I usually pick up quick on how to do things, but this really has me confused. Does anyone know of any sites that can help me figure this out.

First thing you need to do is is find out what the chief complaint is? This goes in the center of the map. Example 79 yr old male hx colon cancer who was admitted for colon resection. Vitals are T 98 P 78 R 20 BP 162/90

Step 2, gather client data. Subjective: ex. Changes in bowel habits. This info should come from your assessment and interview with client.

Step 3, formulate nursing diagnosis. Ex. For colon resection, also see abdominal surgery. When looking under abdominal surgery, the diagnosis may be appropriate for the client. You can choose the diagnosis that best apply to the client based on your assessment. You can also priorities these diagnosis appropriately and number them 1,2,3 etc. ex. For our bowel resection client-nursing diagnosis would include: knowledge deficit r/t self care after surgery, Fluid volume deficit r/t npo status and Ng, Pain at surgical insision. etc.

Once your top 3 diagnosis have been choosen, then you can make your map.Put a box for diagnosis #1 up in the left hand corner, Diagnosis #2 in the lower left hand corner, Diagnosis # 3 in the upper right hand corner of the page. If you find any info that interrelates, then you draw a dotted line to concect & show the relationship between them.

Next step, creating client goals. Ex. Nursing Dx#1. Pt, will rate pain 5 or less on 1-10 scale postop on 1-15-06. your name.

Then evaluate.
