Assessment Sheets?

I just purchased Nursing Care Plans: Guidelines for Individual Client Care Across the Life Span by Doenges, Moorhouse & Murr. Inside there is a nifty adult medical/surgical assessment tool that breaks the body down into Gordon's functional 11 ( which makes careplans easier for me)

I was wondering if by some ray of hope someone has found a computerized version of this sheet. I'm half tempted to create my own based on the model shown in the book, but I figured I would check here first.

Even if you don't use the book, has anyone found a website, or have a list of assessment sheets that you print off and can fill out when doing an assessment? Any help would be appreciated.

It's word based so if you have that program you can tweak it to what you need. Hope it helps.
