When a person falls and hits their head, how often do we do neurochecks?

I know it's q15 for 1h

I work in the Neuro ICU and everyone is q1h neuro checks. Unless written otherwise.

I guess if it is unchanged after that qhx8h then you could go to q2hx84h, then q4hx24h, then q shift.

It depends on what the doctor wants and how the patient is doing. Generally if the patient is stable we will do Q15mx1h, Q30mx1h, Q1hx(either 4h or 8h), Q2hx12h and then Q4h till discharge. If the patient has any neuro changes during that time, or even if the nurse senses something is off, they automatically have neuro checks done more often.

Most facilities have their own policy. They usually all have q 15min x 4, q 30min x 2, hourly x4 and q4h x4. Then they vary depending on whether the facility works 8 or 12 hour shifts.
