Just wondering if someone can tell me a little bit more about incident reporting.... I had a few questions on the nclex today and am still kind of confused about it... does it involve staff, or patient?

Incident reports can involve patients, staff, visitors...anyone.

Anything that has potential for injury or need for

administrative staff to be aware of potential future

consequences, etc. needs to have an incident report

done...they can be called by different titles.

It's also important to know that these are in-house documents (usually for risk management purposes and prevention of future similar incidents) and should not be referenced in your nursing documentation. They are not part of the patient's medical record.
