Hi All,

CT Nurse starting homemaker/companion (non-medical) home care agency. Hoping someone (preferably who has owned or owns a Homemaker/Companion agency) to provide me with some guidance. The home care agency I am starting up will be located in Connecticut. I have already registered, received state tax id, and etc..... website complete. What I need assistance with is Insurance Claim forms to submit to LTC companies? And what forms do I need the client to sign once they agree to accept service from my agency? While I can't suggest Google search terms, there are third party companies that can do back office stuff for you. You may find that much more cost effective than trying to do it yourself.

It is a little unprofessional but forgivable to ask your potential clients to see forms they have signed with other agencies. But since this is non-medical, all you really need is a contract. That is dead simple: just put on it what the expectations of both parties are and the rates. You are trying to get to a meeting of the minds and a contract is just an agreement in writing. You can throw in some stuff about payment policies, perhaps 30 days net, and if you really feel it is important, how disagreements will be resolved. It is your agreement, and you can structure it any way you want.

Perhaps more important is actually keeping track of services provided. Will they be time based? You will want to be very organized about keeping credible records, perhaps even having the client sign a tracking tool at each visit.
