If a nursing home nurse refuses the flu shot and refuses to wear a mask during flu season is it legal for employer to terminate employee in Pennsylvania?

There have been mandatory vaccine laws since 1801. Mandatory flu vaccine isn't new, and there are numerous studies that show it is beneficial for everyone to be vaccinated. There is an estimated 3,000-49,000 people that will die in the United States every year from influenza or influenza related deaths.

There are very few medical reasons not to get the flu vaccine. I have listed what the CDC recommends below, which is overly cautious, since for one a history of guillain-barre has been shown not to have any significance with subsequent vaccinations.

The part about there is no reason for mandatory vaccinations, because visitors aren't required to get the flu vaccine before coming into a health care facility is an ignorant argument at best. Visitors don't normally go room to room seeing several different patients. Visitors aren't volunteering or being paid to take care of patients. Visitors are not charged with ensuring the well-being of the patients. We do ask visitors that are sick to not come into the healthcare facility though.

In the ideal world everyone would be vaccinated, but we can make the most impact to keep ourselves and our patients healthy by ensuring that the healthcare staff is vaccinated limiting the chance that vaccine preventable illnesses do not spread. It has been shown that having a high rates of flu vaccination among the healthcare staff and the residents in LTCs have significant decrease in mortality (up to 55% in the one study).

I also, believe it is unethical to come to a public website and suggest that you can go to a healthcare provider and have a false medical note written to avoid getting the flu vaccine. This isn't a closed website, and anyone can do a search and find that quote. Then they can read further and find that it is from a nurse who states they work in research and has 40 years experience. Not only is it bad advice, true or not true, but it actually endangers people's lives when people are not vaccinated.
