In all likelihood, you will not need to take an entrance exam to enroll in a CNA program. It is not a traditional academic program.

If you enroll in a community college CNA program, you'll probably take the placement exam they administer to all students who are enrolling in the school. Otherwise, I do not see the reason for an entrance exam.

It depends on where you go. There is likely not going to be a formal entrance exam for a CNA program. At the community college they prefer you take Math and English placement exam, but it is not required. (It is beneficial in that they provide extra tutoring and support for those who score low on the placement exams).

I took a short exam to test English language comprehension for the CNA training program I enrolled in. The purpose was to ensure that students enrolled in the course met the level of proficiency necessary for the course material. If a student's English level was too low they could drop the course with out penalty.
