PRC form and requirements for CES Request for Validation of License - Page 4

Hi,

I would like to seek help from this forum. I am a US citizen educated in the Phils. I do not hold a Phils. RN license. I am applying for CES as required by North Carolina BON. Would you know if a CES report will be process without the Phils license? Do I still need to take an English Proficiency exam like IELTS for NCLEX eligibility as a per NC requirements?

I hope somebody could enlightened my mind with this NCLEX and CES application.
