Does anybody know why a person has to pay for quick results on NCLEX? It seems a bit like performing an ultrasound (which a person has to pay for) and then saying - "Oh by the way...if you want to know if its a boy or girl you can pay an extra fee and we'll tell you....

Expedited service almost always results in an extra charge.
I do understand that, but this is (at least in MO) a click of a computer button after purchase, so the information is there whether you choose to purchase or not and is instant after purchase. The hard-copy is not expedited and is still mailed in the normal time frame....
You're not paying for the piece of paper, you're paying for the service. I'm sure a lot of people do pay for quick results, so it makes sense to have all the results available and charge/not charge accordingly. Someone had to set up the program, get the results there, process the payment, etc.
If you wanted to be a "glass half full" type of person, you could look at it as a "discount" off the cost of the test ....a discount if you're willing to wait a bit longer for the results.

Healer, I think the reason for your confusion may be because you aren't realizing just WHO you are paying....and for what. You are NOT paying the BoN in your State for the Quick Results. You are paying Pearson Vue, the agency that administers the exam.
Pearson Vue's contract is with individual State Boards of Nursing, to deliver results to THEM. If you want to find out faster than your BoN is going to otherwise tell you.....Pearson Vue is charging a fee to give you that information as well. They are under obligation to the BoN.....but not to you.....so if you want the additional service, it's gonna cost $7.95.
It's also worth noting that some States do not process background checks, application reviews until AFTER they receive NCLEX exam results. Passing the exam isn't a guarantee of being issued a license, it's just one of the steps. So some States don't give you any information until you have been issued a license. To find out the results of the EXAM, that's available via QR from Pearson Vue.
