Who makes your assignments?

Just had a quick question...

Who makes your assignments, the off-going charge nurse or the charge nurse for your shift?

Charge for our own shift, but the day before. So right now (at appx 0530) the charge nurse is making the assignment for the shift coming on tonight at 1900. If a pt crashes during the day, the day charge will tweak things. Sick calls have to be made 3hrs before start of shift, so usually at the end of a shift the CN will do any adjustments that need to be made 2/2 acuity and staffing changes for the oncoming shift. Charge nurse also comes around at about 4 (day and night) and gets a brief report on each pt so they know what's been going on, if someone's gotten worse/better, if an assignment was too heavy, etc. The charge nurses don't give each other full report until the assignment is out and the staff nurses have hit the floor.

We use a system that is NISS, nursing intervention scoring system which is essentially another piece of paper work. it's a long list of nursing interventions such as foley, monitor up to vent weaning yadda yadda yadda.

But mostly people just poo poo the system so what we do on nights is the charge will round and verify major issues and pass around a sheet for the nurses to rate their pts on a 1-5 system. then assingments are made first according to who was there the day before and points are used to determine who gets the first admissions.
