What's the dumbest thing you've done that you got written up for?

I did something stupid and I think I'm going to get written up for it. I need to hear other people's things they've done that they got written up for. I feel like a putz.

Oh goodness, I have a few good ones. 
 

I was written up last Summer because I just arrived on the unit (10 minutes prior To my shift and not clocked in yet.) with my hair down. I had a HORRIBLE headache and was saving throwing it up until the last second. My charge nurse didn’t even mention it and actually commented on how “long and pretty” it was. 10 minutes after I clocked in she handed me a Write up for a dress code violation. 
 

I’m an agency nurse and a few weeks ago I had a run in with a charge nurse (that’s a story for another day.) but essentially she lost her mind in me for asking a simple question. I was called numerous names (the C word and “retard” being two of the worst.) 2 weeks ago I had a meeting to discuss why I wouldn’t pick up there anymore and that incident. I was then written up for failing to report it In a timely manner because I could’ve caused the other patients emotional distress and possibly abuse by the other nurse.

WOW!! I hope the nurse who called you those horrible names got written up too.  

Here's mine.  Like you, I get bad headaches.  Migraines.  I was an agency nurse, scheduled for a 3-11 shift.  I took imitrex before reporting for my shift. It works within 1-2 hours and I took it around 2ish so I wasn't too concerned.  During report, I was holding my forehead, because my head hurt. I told the reporting nurse, "I am listening to you, I just have a horrible migraine and I'm waiting for my medication to kick in."  Which it finally did. Of course she was fine with that. OMG, you poor thing, that must be awful. She was so sympathetic. Ha ha. Well, then, the END OF THE WORLD happened, LOL.  I got a call from my agency, I was written up because I forgot to change 1 patient's Duragesic patch.  I changed like 4 of them, plus like 20 lidoderm patches.  But, the reason I forgot her patch, according to the nurse, was because I wasn't paying attention to her report. She told my agency that I was just sitting there with my eyes closed and seemed like I'd rather be ANYWHERE else but there. I was like, "SERIOUSLY?? That's what she said?"  I so wanted to confront her. It wasn't like that at all.  I was like, she KNEW I had a migraine. I was soooooo mad. LOL

I once got in written up because I was the only nurse who didn't lie about a patient's constant refusal to take his meds. I mean, he would really fight and I documented how I made numerous attempts to get him to take his meds, but to no avail. All the other nurses who worked this assignment simply dumped the doses and acted as if there were no problems at all. Meanwhile, when I was re-assigned, a new nurse tried to give him his meds and documented the refusal & fighting (just as I had). 

My supervisor quietly "dropped the charges" but never apologized... in fact she assigned me to the worst, most difficult sections of the whole place! She remained very offensive to me from then on!
