When you talk to the nurse taking care of your patient in a facility and she says "I skipped patient's last dose of scheduled morphine and lorazepam because they didn't need it". This is they patient who is actively dying and has a black foot with gangrene.

Most people writhing in pain are breathing more rapidly. It's not easy to have a hard and fast rule about an end of life issue.

Not the lady I had on Saturday. Her resps were 8 and in extreme pain.

Sometimes it hurts too much to breath and patients tend to hold their breath. The pain medicine could help them breath.
