What do you think of JCAHO?

I keep hearing more absurd edicts from the joint commission. They've banned certain commonly worn shoes called crocs. They've banned pens that go around your neck. They also seem to have many other annoying and time consuming requirements. I think they are ruining bedside nursing. They seem to think that the more forms filled out, the better.

Yesterday I had a vent pt with restraints. We have a stupid restraint form that needs a million boxes filled every hour. It totally loses it's meaning and does nothing to improve care, but my manager has to worry more about this form than a million more important things. She's always reminding us to fill in the million boxes. I just check them off ahead of time. The pt had many other more important things going on, being on several drips, Q 1 hour blood sugars, and I was taking off orders all day long.

Anyways, I think the regulators have become a self-perpetuating bureaucratic entity in my opinion.

Okay I am in trouble because I am honest. 1st thing I want to tell the evaluator who will come to me with all these questions is first. You know that not one nurse is glad your here with us. We have enough pressure in our working conditions than to have you watching and over looking our work. Because your here right now I am not performing my usual care. Which by the way of my staff and patients is Excellent? I will be so afraid to make a mistake that I will second guess all my duties and procedures which in turn will have me losing confidence and make me have issues with my self worth and then presto you have a nurse of 25+ of success, who has most likely more floor hours and pt hours than you have feeling on the verge of " Hell with this and then question why I ever got into this profession" I still love nursing, you know the old act of caring for the sick, until I have to be worried about all this other stuff like promote you Hospital above all and "your patient is a customer" stuff. My patients are folks who are ill and need care. Let us give them the care and compassion like we want to and you will never have to pay 5x more than my salary on TV and radio adds. Let nurse nurse and you will not feel the shortage. Learn to retain the ones you have and you will not have to lie and candy coat to hire nurses. How to I relate these problems with JCAHO. Simple the ties with insurance companies and Medicare and other agencies have caused the decline in healthcare because with the other opportunities folks have the people who would become Doctors, nurse and other fields of healthcare. Avoid the field because of what JCAHO have forced it to become. Now for a statement that I am sure will cause some to turn me off. But I rather see a Government method of regulating healthcare, than a profit business like JCAHO. So many folks do not realize the JCAHO is a business who through their own self promotion have made themselves so important and pulled the wool over the eyes of the consumer as well as the public. Sorry I just had to say this..

People say "thus and so is a joint commission requirement." They do this to advance their own agenda. Joint commission is not much different from AM best. They simply make sure you are following your own policies. If you check their website you'll see that they make recommendations. About everything in those recommendations is reasonable and good nursing practice.

Don't get me wrong, I am certain that the joint commission causes more harm than what little good it does and is more representative of a medical model than a more appropriate nursing model for hospital care. But most of the stupid stuff attributed to the joint commission is not really from them.

What I think about JACHO is not printable in polite company. It also doesn't matter one darn bit. They are what they are. I avoid them if possible, if not I just try to be nice and deal w/it.
