What questions are asked at intake evaluation

I would greatly appreciate any feedback on what’s asked at an intake evaluation. I’ve been reading that these monitoring programs will send you to multi- Evals if you “pass” or are determined to not need to require the program. No mental health hx, all my drug screens are negative. I’m under investigation at my hospital for improper waste, we use old school paper charting.
