Anytime the lab is calling with a critical lab value, the nurse should use a read back method along with providing their name, and then notifying physician and documenting

I am not sure I really understand your question about the medication change. Patients and family should always be educated about any medication, especially medication a pt will be d/c'ed with.

It will vary where you work. At my hospital you read back the result and they ask your full name for their records.You owuld then document that you were notified of a critical value and let the doctor know.

If a family member asks about someone's meds you can explain the medication change as long as they are the POA.Where I work only the POA can be given patient information.
