Veteran Affairs (VA) Case Management

Hello, does anyone work as a  Case Manager/Telehealth with the VA (Veterans Affairs)?

If so what does your day consist of? What are your job duties...do you have to make appointments, connect veterans with resources, etc? Do you enjoy your job? Thanks

I am a case manager with the VA. I do the same for veterans as I would my own civilian patients.  Within the VA the big focus is 30 day readmissions.

Hi-are you a telehealth case manager, if so is your primary responsibility keeping the veterans from readmissions. Do you reach out to the veterans via phone and over the computer?
