Vancomycin trough

I have worked at a few different places, and the facility I'm currently at does vancomycin different than any other. I'm curious as to how you do it. Which is correct when drawing trough prior to every 3rd dose?

We draw troughs before the 4th dose. Plus Renal function labs every 48 hrs
