Let me hear how you guys perform Valsalva maneuver?

Interesting application of the valsalva :) I wont be checking the dura anytime soon in my patients (or when i do its because the cranial bones are gone)

Ever wonder why your obese or maybe not-so-obese patients, particulary your GYN or abdominal patients are hypoxic after being in Tberg with abdominal insulflation?
Good ole atelectasis. Most of these categories of patients get a valsalva from yours truly prior to extubation.

One more thing......valsalva manuever can lower heart rate.......do you guys refrain from using it with patients that are on parasympathomimetics or drugs that decrease automaticity. (Just people that have a lowered heart rate in general?)
I can just see someone in to get a pacer for their heart being paced by the AV node, someone putting in a central line like mentioned above and sending them into severe bradycardia?
I could be talking out of my rear, that's why I'm asking?

You are correct this absolutely happens. I have found, in my experience anyway, that these patients respond so quickly to atropine that it hasent been an issue for me.
