VA proficiency Nurse II or Nurse III

I am a ICU nurse that has been working at the VA for 1 year on a contract assignment. I applied to become a permanent employee and received my initial offer letter. I was displeased with what they offered to say the least. I spoke with my manager and was told I should submit my proficiency (NPSB) with my letter to decline the offer. Problem is I have never wrote a proficiency before. I have my MSN and have years of ICU experience in the private sector and my experience with the VA. I have joined multiple committees, started unit projects, and continued working on my education/ leadership skills while working at the VA for a year. Does anyone have a proficiency for Nurse II or Nurse III they could share with me as an outline. I am lost on how to write this and need help. My coworkers are unwilling to share theirs and management is not helping. PLEASE any help would be greatly appreciated.

You can do a Google search for VA nurse II or nurse III proficiency.  In a nutshell: Write what you saw was needed, what you did (really blow your own horn!), the outcome, and is it sustainable?
