Hoping someone can help me. I have a few questions about UR meetings so here goes:
How do you run your UR meeting? ie:does PT or nsg run it,how long does it take?do you bring charts and if you do just the ones that are about to come off therapy or nsg or all MCR charts?
Do you use a form for the nsg info? how often do you update it? ie: daily in clinical meeting,weekly etc...
then how often do you let the team know about the nsg skills? just at UR or daily etc...
Sorry for so many questions we are just haveing a feww issues and Im trying to fix them and get a good strong system working so any help will be appreciated

In all the facilities where I have done CM, the UR meeting is run by me, whether I'm wearing the MDS hat or the CM hat. The rehab manager and social services must attend. Lots of times my other attendees include the administrator, the director of nursing, activities, dietary manager, and any other members of the interdisciplinary team. Rehab usually has a special form with notes, social and the others bring relevant information. All skilled patients are reviewed. If you have a doctor or other provider attending the charts and extra phys. order forms are brought for obtaining orders. Unfortunately, with 40 skilled patients at any given time, I can't bring the charts, but the administrator wants me to. (Not badly enough to buy me enough racks to bring them all of course.)
I have a form that I use, all disciplines attending make their notes on the form, and sign it. It is NOT a part of the permanent record, but is used for faxing to managed care if needed, as a part of the weekly update. When I use this form, I do not get any requests for "YOU have to use our form," because they will all tell you this in the beginning. Use my form and you won't have that, at least I haven't. feel free to send me a message with your e-mail, and I will send you a template.
