I am under investigation by the NCQAC for falsification of records on a patient I don't remember. It was completely unintentional what I did but I'm so scared I'm gna lose my
liscense now. What should I do?

Falsification of records, even unintentionally, is a very serious matter. AllNurses can not give legal advice. You need to talk to an attorney. You can find attorneys versed in dealing with nursing issues here:

An attorney can't guarantee they'll save your license or even that they can lessen any damage. But you need all the help you can get right now.
If you have malpractice insurance, you better contact them as well and tell them what's going on.
Last, I strongly caution you against discussing this on the internet and social media. Both are forever, and once you post something, you can potentially lose control of it. Even if it's up for a few seconds, during that time it could have been screen-capped, copied, forwarded, printed out, cached, etc. and is now out of your hands.
