My trend word in nursing for 2015 is care transition.

I have noticed that there has been a lot f discussion about are transition in the local hospital, in LTC facilities, and the agency I work for. When patients transition from one setting to another they are more vulnerable to experience problems related to that transition. I have talked a lot about care transitions and noticed that other care provider as well as insurances are taking steps to ensure the care transition improves.
