Transition from Hospice to Hospital Case Manager

Hello allnurses,

I am currently an admission nurse for a hospice company. I have been a nurse for 12 years in multitude of different specialties. I am getting burned out with my current role as an admit nurse. The territory is huge, driving 120+ miles a day. 5x week. Sometimes working 12+ hrs a day including charting. The emotional factor is also finally starting to get to me. Anyone who has worked hospice and now works as a hospital Case manager or patient care coordinator. Please enlighten me on how the transition went. I am eyeing a job at Kaiser hospital in the bay area of California.

Have you made any decisions yet? Inpatient case management is a big switch from floor nursing or home health. The scope of practice is quite different.

Caron have you worked in both environments? I have not made the decision yet but I have started to apply for RNCM inpatient roles. Please enlighten me on the scope.

My speciality is hospice from CNA to RN. I left as an administrator a year ago and went to the VA as a home based primary care case manager.

Your experience in hospice is extremely valuable in any case management position. Because of the uniqueness of our holistic thinking you have an edge as far as meeting and exceeding the standards of case management for your patients.

But it will be different. You have to learn how to think in a way that allows you to let go of your patients in ways that you wouldn't in hospice. But just knowing your background I do believe and know that you can transition and do well in hospital case management.
