Looking for policies regarding the trancription of medications onto MARs. When an ULP transcribes meds, the RN verifies the transcription and DOSING. When a RN transcribes, are they responsible for checking the dose, as well as the second RN? What does your facility suggest? I work in a Pediatric hospital and this is becoming a major debate for us.

We transcribe all our own orders, but we have to have a second RN check the MAR and our calculation sheet. You must also verify the original order when the new MARS come from pharmacy, but this is a single RN check. On our peds floors, it is the same even though the secretary does the orders.
This will be changing once we are to a full computerized MAR since as soon as the order is written it will go to pharmacy thru the computer program, but it will still require a second RN check for computation on all meds and drips.

One nurse transcribes our orders. A second nurse verifies they are entered correctly and the one verifying is the one who actually notes the orders.
