transcribing Triangle Symbol in Dr orders?

I have an assignment for IV Transfusions showing a triangle symbol Gentamycin to Flagyl in the Dr orders. I understand the triangle means change but how to transcribe this to the IV Therapy on the Kardex?

Please let me know!!!

You wouldn't transcribe that "change" symbol. You'd erase the gentamicin and replace it with Flagyl, changing the dose accordingly of course. Basically what you're doing is stopping the gentamicin and starting the Flagyl. The physician has simply written one order instead of two.
