Remember your ABC's and Maslow's pyramid and that will help your prioritize any diagnosis.
1. Do any of your diagnoses affect the airway? Example: Ineffective airway clearance. That's 1st priority.
2. Do any of your diagnoses affect breathing? Example: Ineffective breathing pattern. Those are next.
3. Diagnoses affecting circulation. Example: Decreased Cardiac Output. Circulation problems are next in priority.
4. Maslow step 1: Physiologic needs. Food, Water, Warmth, Sleep, Elimination, Shelter, Pain
5. Maslow step 2: Safety and Security.
6. Maslow step 3: Love and Belonging.
7. Maslow step 4: Self-Esteem
8. Maslow step 5: Self-actuaization
Since all of your diagnoses can have different pathophysiology, I can't prioritize them for you. Look at your diagnoses, and the assessment data behind them (your related to section) and decide which of the above categories they fall under.
Also think about which ones may affect the others. For example, activity intolerance and ineffective tissue perfusion may effect impaired skin integrity. Also look at what the patient is in the hospital for. If your diagnoses have to do with the main admission problem, that could be a priority. Without knowing the patient we can't really tell you which is the priority because it's based on the patient.

So i have got decent at putting together the diagnoses my teacher wants each week, however i still have trouble gettin the priority order right especially when it is like this weeks diagnosis'....... they are impaired skin integrity, impaired tissue integrity, hyperthermia, activity intolerance, and risk for ineffective peripheral tissue perfusion....... i know the risk for is last but what order should the other for be in?
