Titrating insulin

Hello,

I'm a remote RN Care Manager who's been now tasked to titrate patient's GLP-1/ insulin combination medication weekly based on the patients verbalized blood sugars. This will be a standard order for my company. The doctor will write the initial dose , however this will be nurse driven and the nurses will be responsible to increase the insulin weekly for a few weeks. Has anyone heard of this practice? Thanks
