morning...just wondering what you all think of this.....as you know,i work in a 12 bed IRA house for medically frail mentally challenged individuals......
this is it......the other day i go to work,and we have a communication log for staff,to let us know of med changes,new dr orders,etc......we are all LPNs that work there.....we now have to call a triage nurse to give tylenol for increased fever......ALL our people have standing PRN orders for,tylenol,codiene,morphine,diastat,plus more......but because the RNs in the office have not been following up on our reports of change in status(increased fevers) we now have to go thru this...which means....calling the triage nurse,asking her approval.....faxing the MAR and history to them.....oh it is such BS....why oh why did i become a LPN.....oh i know....to get babysat!!.....
so yes...i guess the carpet people(office people)got their hands slapped cause this one guy would have a fever a couple of days.....be good for a few,then it would go back up.....same with his BS....it was high for several days in a row.....iknow....cause i had to call the dr 3x in one day...protocal is..if BS is over 400...notify MD.....that is on the weekend when no rns are in the house.......anyway....we HAVE been telling the RNs about the fevers,and the BS....we have a 24/hr report we write EVERYTHING on....plus the log....they read it whenever they come......we have told them something is brewing with so and so....please order some test.....we know our people.....we see the changes.....
so what i am asking....and mostly venting cause there is nothing anyone can do about us calling triage......what do you all think about this......every single staff member is PO
oh forgot to mention.....we still can give cod and morph at will.....one girl can get codiene for her autostorming q2hr......go figure

What this represents is that the RNs did not do their job, it is not really a reflection on you all; but BECAUSE they didn't do what they were supposed to do, this corrective action is supposed to light a match under the RN contacted and have them intervene. It would not surprize me if it goes back to business as usual in a few weeks or months.
In the meantime, contact them each time and make them see that this makes no sense.
