I'm having a problem creating a questionnaire based from our statement of the problem.

our statement of the problem focuses on the diff. terms like the

1. profile of the respondent which includes their age, Edu. attainment etc.

2- factors affecting

3. problems encountered

4-5 significant relationship of 2 variables

6 plan can be proposed

our topic is all about the newlywed/ would be couples in the nurturance for psychosocial development of children.

Can someone give me some tips or suggestions :3
