What are your thermoregulation guidelines?

Do you go by weight to determine the need to use servo control versus manual control in the infants isolette.

What is your policy/guideline for humidity.

We NEVER use manual control unless we are weaning the baby out of the isolette, way too much risk of overheating the baby, we also never use manual control on our warmers. Preemies stay in the isolette on servo mode until they are 1.7kg, then we can start weaning them IF their internal temp has been less than 30c for the last 24 hours, we wean over 48 hours, if they pass they can go to an open crib, if they fail at any point they go back on servo mode and the test cannot be redone for at least 48 hours

Humidity is used for infants 28wks and less, 60% for two weeks then we just turn it off.

There is no official weight or gestational age for changing from Servo to Air control where I work. There is a "guideline" for weaning to open crib. It is decent weight gain, >34 wks AGA and >1500g. In reality we rarely wean out of isolette before 1600-1800g.

Though we don't have a guideline for changing to air control, it is generally done at >1400g. Of course this is with the infant being stable. Often if the infant is IUGR or SGA and wasting a lot of energy flailing, we might swaddle and leave on air control a little earlier.
