I personally loved Clinical Nursing Skills and Techniques by Perry and Potter. There are numerous editions. Our first nursing text book for fundamentals (not me surg) was Basic Nursing, also by Perry and Potter. Now that book, I honestly didn't read through all that much of it. What was necessary to learn was taught in class, and when I wanted clarification/a mind jogger on I would then refer to my text. That book has some basic clinical procedures with rationales for each step, but for skills, the first book I mentioned was very comprehensive with both beginning and more advanced skills

We used Potter and Perry. Our school used the same book the hospital makes policies and procedure from. I liked the book.
