Our policy (and likely most) do not get into specific procedures. However, it talks about whether or not a cavity is entered. If the cavity is entered (ie, the peritoneum is opened), then we are required to do an instrument count.
I notice that this is not the first time you've come with a count question. Perhaps you would benefit from sitting down with your OR educator or leadership to clarify your count policy.

What procedures do your facilities omit instrument counts? I know counts are done for sharps and softs on all cases, and instrument counts vary.
More specifically, what is your policy on procedures such as a radical orchiectomy inguinal approach? Inguinal hernia? Procedures where there may be a conflict of thought process.
