I just started back in an LPN to RN program. I have to do to diagnosis on the following scenario and any suggestions would be appreciated.
Female 82 just admitted to nursing home after short hospital stay for malnutrition
Living independently since spouse death 10 years ago
3 months ago after bout of flu her appetite and strength diminished
20# wgt loss led to hospitalization
3 sons that visit regularly and supportive of her return home when her strength improves and nutritional intake is stable
family describes her as extremely sociable nurses have observed interacting very little with others and spending most of the time in her room
"I don't want to bother other people by asking them to repeat things I can't hear" hard of hearing
Alert, oriented medical dx osteoarthritis and malnutrition vital signs stable no edema or SOB, no smoking hx or hx of ETOH abuse appetite fair to poor unsteady gait and balance is poor during transfers from W/C to bed. Able to bathe and dress self. Skin is intact but fragile.
I am going to start working on this but will check back for replies.

There are tons of NANDAs to work with! Show us what you got first, then we'll help you from there. I hope you understand this stems off many people coming here looking for answers and not thinking for themselves. It's nothing personal, I just think giving guidance over giving answers works better in the long run.

I think I got it figured out. I am using Disturbed Sensory Perception related to auditory deficit and Imbalanced Nutrition: less than body requirements related to recent weight loss. I am turning them in tomorrow.
